You had joined [insert or write the name of the clinic] on [insert or write the joining date]. It had been a quite long time over here for you. You had been given enough scope to learn the job as Phlebotomist. But you could not prove your ability.

Since you were a fresher and so we got you assigned under the supervision of [insert or write the name of the senior phlebotomist]. You could not learn the skills from him to arrange and label the blood samples with numbers.

You were not confident working independently. We saw your hands trembling with nervousness while doing the venipunctures for collecting the blood samples. Also, you were not found comfortable doing the other diagnostic procedures. You need to have a strong nerve to do the tasks of blood transfusions and blood drawing procedures.

Two days ago, you did a big blunder. You did not see while collecting the blood test samples from two patients suffering from typhoid and jaundice. Unmindfully, you labeled the blood sample of the jaundice patient as a blood test for typhoid.

It is because of your blunder, we had to call those two patients again for getting the blood test done. These actions are completely unacceptable and no lab or clinic will accept this kind of blunder which can even amount to taking away patients’ lives.

You have completely proved to us that you are unfit in the profession of Phlebotomist. Either you change your profession or do it carefully. We cannot afford to keep you in service over here.
