Dear [Insert or write the full name of the recipient],

We regret to notify you that your job as a pharmacist will come to an end on [insert or write the date of termination]. After then, you’ll no longer be employed by [insert or write the name of the pharmacy].

The reason behind your termination is your poor performance. We trained you and told you that to keep your job at [insert or write the name of the pharmacy]; you’d have to improve your performance.

If you did not improve your performance within one month, you will be fired. After one month of the notice period, you did not perform up to the standard expected from you.

Therefore, your employment has ended. As a pharmacist, your job was to compound and dispense medications, as prescribed by physicians, and instructing customers on how and when to take prescribed medications.

You were also asked to complete operational requirements of the pharmacy, including records of controlled substances, verifying order entries, maintaining charges, and removing expired and/or damaged drugs from the pharmacy’s inventory.

You will receive the severance payout on your last day of employment. Your final paycheck will be delayed if you fail to return the property of the pharmacy that you have in your possession. If you have any further questions regarding this letter, contact your manager.

Thank you for your service. We wish you all the best in your future endeavors.

With regards,
