I am right now admitted in the hospital and under doctor’s guidance. I will be undergoing surgery within a week. I have been strictly prohibited to move anywhere for next 3 months.  

For these above-mentioned reasons, I am appointing Mr. /Ms. [name] for signature-related responsibilities. He/ she will be holding the position of authorized signatory for a period of next [number] months. He/ She will be holding this respective authority till [ending date].
