If any situation occurs when your family doctor is moving to a new place or taking retirement from his/her profession, then it is vital to bid him/her goodbye with a farewell letter for providing his/her service for many years.

We have given some sample farewell letters here for your convenience. In this letter, you should mention the number of years he/she has treated you, the name of the doctor, and your address.

You can also mention how he/she has cured you of critical health conditions and given you the right advice at the right time.
