You were informed at this meeting that your [conduct, behavior, or activities] were unacceptable and that immediate improvement is necessary, or that [conduct, behavior, or actions] must not occur again. You were specifically informed that [description of undesirable conduct, behavior, or activities, such as frequently arriving at work late or using inappropriate language/completing necessary chores].

You were asked in the meeting if you had anything to say or how you would react to the issue, and you [include details of your response, or if none was given, record that you did not respond].

After taking into account your responses and explanations, I came to the preliminary conclusion that your [conduct, behavior, or actions] needed to improve. Specifically, I recommended that you [give the employee instructions regarding improving conduct, behavior, or actions, for example, outline expectations (make sure whatever you demand is reasonable and fair in the circumstances and that you have taken into account any response or explanation from the employee)].

If [your conduct improves/you do not repeat the behavior] at that time, this warning letter will be ignored for disciplinary purposes after a period of [a fair period of time depending on the behavior].

Dear [Mention the name]

You failed to show up for work at [Mention the time] on [insert date] as scheduled, and you did not notify your manager in advance that you would be absent. Furthermore, when you came back to work on [insert return date], you didn’t give a good reason for your absence or explain why you forgot to give your boss advance notice. Your disregard for the organization’s attendance policy is inappropriate and will not be accepted by [company name ], as stated in the employee handbook and as previously conveyed to you.

I let you know my initial opinion, and [insert specifics here if you didn’t or didn’t answer]. This warning is being issued as a result of my decision after carefully weighing your [response or lack of reaction] to my initial assessment.

This is your [first/second] letter of warning. If your [conduct, behaviour, or actions] do not improve sufficiently [or if the action is repeated] by [date that is fair in this situation], your job may be terminated.

According to an examination of your employment history, you have absented yourself from work without permission or failed to give a valid excuse on [Mention the number] occasions in the most recent [Mention the number] months alone. These were missed days, [Mention the dates]. You received verbal warnings on each of those times and advice to give adequate notice of any absences. You could face harsher punishment, up to and including termination, if you don’t give the required notice of subsequent absences.

I’m grateful.

Dear [Mention the name]

As we just discussed, your [Mention the absenteeism] is detrimental to both your team’s productivity and your personal effectiveness at work. We take [workplace attendance] seriously, and we anticipate that all of our employees will [adhere to the company’s vacation policy].

However, despite warnings, you have repeatedly disregarded [Mention the rule] and [have gone over the permitted amount of absence days]. To be more precise, [in the previous month you skipped work on [Mention the date], without informing [Mention the job position] or your manager in advance.] We [are flexible for instance, by allowing the opportunity to work from home or take sick leave to take care of a sick family member because we do recognize that unforeseen situations may occasionally develop. However, we still demand that they conduct themselves professionally and behave in the interests of the business.

This is your [first] note of caution. We anticipate you to behave better and request time off formally, in advance, by [Mention the email address] to your boss, or through our [Mention the name]. If this doesn’t happen, we’ll have to impose more punishment, even firing.

I’m willing to talk with you about any issues that led to your [absence from work.] If there’s any way I can assist you, kindly get in touch with me directly.

Sincerely,

I’m grateful.
