I (Name of the employee) sent this letter on the behalf of our health insurance company(Name of the company) regarding request number 1278387 which is sent by you to our email address.

We are sorry to tell you that our company has rejected your request for the health insurance claim that you had requested. The surgery that has been done on you is not covered by any health insurance company.

Your surgery comes under the category of ‘cosmetic surgery’ which is done for the sake of enhancing personal beauty. Our company only covers the surgery which is done for any illness in the body.

With this letter, I also want to inform you or educate you more about the company’s terms and conditions. Any treatment which comes in the category of low injury is also exempted from our health insurance policy, maternity care is also not included in your policy. It is required for continuing the insurance with our company for at least 2 years for getting the maternity cover.

A person who is seriously ill should be admitted for a minimum of 24 hours in the hospital and then with the letter of the doctor who is assigned for the illness cure should be submitted to our company’s office to issue the money for the treatment.
